    In case of an incident covered by the insurance agreement, before you take actions on your own you should contact Centrum Pomocy open 24/7, calling the following telephone number +48 22 575 90 80.

    When contacting Centrum Pomocy the Insured or person acting on his behalf should:

    • Explain in detail to the employee of Centrum Alarmowe the current circumstances of the Insured, what kind of assistance is required and state the necessary information about the insurance, i.e.:
      • insurance document number (Euro 26 card)
      • name and surname of the Insured,
      • telephone number at which Centrum Alarmowe may contact the Insured or his representative.
    • Provide the doctors from Centrum Alarmowe access to all the medical information,
    • Comply with the recommendations of Centrum Alarmowe, providing information and necessary authorisations,
    • Enable Centrum Alarmowe to perform actions required for determination of the circumstances of the loss, rationale behind amount of the claim, and provide for this purpose assistance and clarifications.

    If written report of the loss is required, complete documentation identified in the  Detailed Terms and Conditions of the Insurance should be sent to the address of the loss adjustment company acting on behalf of AXA TUiR S.A.
