  • Claims filing

    Step 1: Report the incident to the police for written confirmation of your report within 48 hours (in case of unauthorised card use, cash robbery, purchase of goods).

    Step 2: Fill out a proper loss report form.

    Step 3: Collect the necessary documentation along with a loss report form (the list of documents can be found in the Detailed Terms and Conditions of the Insurance and it differs depending on the specific types of insurance within the package).

    Step 4: The Insured is under the obligation to submit to the Insurer documents confirming occurrence of the incident covered by insurance protection within 60 days at the latest, counting from the moment of occurrence of the insurance incident.

  • Communication with your Insurer

    You may resign whenever you wish through submission of your statement of resignation from Pakiet Bezpieczna Karta insurance at Bank Millennium’s branch, in writing, or  over the phone at TeleMillennium.

    Should you submit your written resignation from the insurance, your insurance protection under a given card expires on the last day of the calendar month, for which the last insurance fee was collected.

  • Insurance resignation

    You may resign from the insurance whenever you wish at Bank Millennium’s branch by signing the Application concerning card insurance products or over the phone at TeleMillennium.

    If you resign from the insurance, the protection for the Insured expires:

    • In case of work loss insurance – on the last day of the settlement cycle for which the insurance premium was paid.
    • In case of accidental death insurance - at the end of the settlement cycle, in which resignation from the insurance protection was submitted, provided it was filed at least 10 days before the elapse of the settlement cycle, or at the end of the next settlement cycle, provided the resignation was filed without this notice.

    If when staying abroad, you have an accident, sudden onset of a disease or in case of another incident covered by the Assistance insurance, then before you take actions on your own, you should contact within 24 hours Generali’s Centrum Pomocy open 24/7, calling the following telephone number 0 22 205 51 94.

    When contacting Generali’s Centrum Pomocy the Insured or person acting on his behalf should state:

    • Type, number and expiry date of the Insured’s card,
    • Name and surname of the Insured,
    • Explain in detail to the officer on duty the current circumstances of the Insured,
    • Define what assistance he needs,
    • State the telephone number, at which Generali’s Centrum Pomocy may contact the Insured or person acting on his behalf,
    • Provide the doctors from Generali’s Centrum Pomocy access to all the medical information.

  • Insurance resignation

    You may resign from the insurance through submission of statement of insurance resignation at Bank  Millennium’s branch, in writing, or over the phone at TeleMillennium.

    If Card holder submits his resignation from insurance protection under the International travel insurance, insurance protection expires at the end of the calendar month, in which the Card holder submits his resignation from the insurance protection, however, resignation can be made after elapse of twelve  months at the earliest.
