    Step 1: Fill out the benefit payment application.

    Step 2: Collect the required documentation together with your benefit payment application (for the list of documents see the Terms and Conditions of the Insurance).

    Step 3: Submit the required documentation to the nearest Bank Millennium branch.

    Remember that you may also submit the required documents directly to the Insurer (TU Europa S.A. for Temporary Incapacity to Work Insurance, Accidental Death Insurance, Permanent Income Loss Insurance, Hospitalisation Insurance, or TU na Życie Europa S.A. for Death Insurance, Total Incapacity to Work Insurance, Disability Insurance, Serious Illness Insurance, respectively) in one of the following manners:

    • in writing to the address: ul. Gwiaździsta 62, 53-413 Wrocław,
    • through the TU Europa’s communication tool at www.tueuropa.pl website,
    • on TU Europa S.A. Infoline at 801 500 300 or 71 36 92 887 (from Monday to Sunday between 7.00 and 22.00),
    • in person or through your representative at the Insurer’s head office.

    TU Europa S.A./TU na Życie Europa S.A. reviews the claim within 30 days from the date of receiving notification about occurrence of insurance event.

  • Withdrawal/termination of the insurance agreement

    The Policy Holder may withdraw from the Insurance Agreement through submission of his withdrawal statement within 30 days from one of the following dates: the day of concluding the Insurance Agreement or start date of the Insurer’s liability, whichever later (provided that there have been no triggers for payment of the insurance benefit). In such a case, the Insurer returns the full premium.

    The insurance agreement may be terminated at any time through submission of statement on agreement termination. In such a case, the Insurer returns part of the premium for the unutilised protection period. The returned costs of the insurance protection reduce the amount of the loan to be repaid.

    Withdrawal/termination of the insurance agreement may result in the change in the loan interest rate in accordance with the Price List.

  • Claim reporting

    Damage or claim should be reported via the damage report form:

    • from the Insurer electronically on the website www.tueuropa.pllink opens in a new window,
    • in person or in writing to the agent of Bank Millennium S.A. or from the agent of Europa Millennium Financial Services sp. z o.o.

    The claim is considered within 30 days from the date of receipt by TU Europa of notification of the insured event.

    Pursuant to the General Terms and Conditions, notification of the event must be submitted to the Insurer no later than 3 days from the date of occurrence of the damage or obtaining information about it.

  • Withdrawal/termination of the insurance agreement

    Building and residential premises insurance constitutes loan security, so waiving it is possible if the client:

    Note: If the loan agreement does not include a provision on the possibility of individual insurance, changing the insurance requires an annex to the loan agreement. Moreover, in order to cancel the insurance, you must first apply for insurance waiver at a Bank Millennium branch.

    • purchases insurance from another Insurance Company that is on the list accepted by Bank Millennium, and
    • after the Bank accepts the insurance documentation and accepts it as security for a housing loan, mortgage loan or consolidation loan, the rights arising from the insurance contract will be transferred to the Bank based on the assignment.

  • Claims filing

    In order to file a claim the following information is required:

    • CIF number of the borrower for whom the declaration was registered,
    • date of credit disbursement.

    Losses or claims should be reported directly to PZU SA in one of the following ways:

    • on-line, by filling out a form posted at www.pzu.pl (documents may be attached to the report),
    • by calling the PZU SA’s Info line at 801 102 102 or (22) 566 55 55,
    • personally at any branch of PZU SA.

    PZU SA transfers the indemnification information to Bank Millennium. Then, the Bank takes a case-by-case decision to transfer the amount for credit payment to customer’s account. In the next step, the Bank informs customer about the transferred indemnity by way of statement of the personal account.

    Indemnity is paid within 30 days from the date when PZU SA receives notification about the insurance incident. If the amount of indemnity does not exceed 5000 PLN, it is always transferred onto the customer’s account. If the amount of indemnity exceeds the current credit debt, the Bank transfers the remaining part onto the customer’s personal account.

  • Insurance resignation

    Building structure and residential insurance constitutes credit collateral and therefore resignation from this insurance is possible, if customer:

    • takes out insurance with another Insurance Company that is included on the list of Insurance Companies accepted by Bank Millennium, and
    • if, after the Bank accepts the documentation concerning the insurance and adopts it as security for the residential loan, home equity loan or consolidation credit, the rights from the insurance agreement are transferred on the grounds of the assignment.

    Note: If the credit agreement does not stipulate the possibility for individual insurance, change of the insurance requires annexing the credit agreements. Furthermore, in order to resign from the insurance, one should submit application for resignation from the insurance at Bank Millennium’s branch.


Life insurance (from 02.12.2023)

  • Claim reporting

    Step 1: Fill out the Event reporting form in accordance with the guidelines.

    Step 2: Collect the remaining required documentation that differs depending on insurance event, along with the Event reporting form (the list of documents can be found in the supporting event notification/benefit payment application form available from the agent of Bank Millennium S.A. or from the agent of Europa Millennium Financial Services sp. z o.o.).

    Step 3: Submit the required documentation to the nearest Bank Millennium S.A. branch or at the Europa Millennium Financial Services sp. z o.o. agent or electronically on the website tueuropa pl.

    The insurance benefit is paid/implemented by TUE na Życie S.A. within 30 days from the date of receipt of the notification of a given insured event.

  • Withdrawal/termination of the insurance agreement

    According to the T&C, the Policy Holder has the right to withdraw from the insurance agreement within 30 days from its conclusion and to terminate this agreement at any time during its term.

    Withdrawal/termination of the insurance agreement is performed through submission of the Statement on withdrawal/termination of the:

    • to the Agent in person, by telephone or in writing, or
    • to the Insurer in person, in writing or by e-mail to bok@tueuropa pl. If you wish to send the declaration by e-mail, please register your e-mail address in advance at the Insurer's Customer Service Office.

    Life insurance constitutes security for the mortgage loan and therefore in case of withdrawal/termination of the insurance agreement, the client is under the obligation to ensure security for the residential loan/home equity/consolidation loan through assignment of rights under the life insurance agreement concluded with another Insurance Company accepted by the Bank and fulfilling the minimum insurance coverage required by the Bank (life insurance).

  • Insurance resignation

    If you resign from the insurance, you should:

    • submit your statement of insurance resignation at any Bank Millennium branch, or
    • send statement of insurance resignation to TU Europa S.A. to the following address: ul. Gwiaździsta 62, 53-413 Wrocław, Poland.

    Your resignation statement should contain the following information:

    • insurance referred to in the resignation,
    • data of the person who resigns from the insurance,
    • credit account number,
    • date and signature of the person making the statement.

    Note: The customer is entitled to the refund of the insurance fee for the unutilised period of the protection.
