Family 500+ application - frequently asked questions

  • 1. Does Bank Millennium support filing Family 500+ application?

    Family 500+ online applications are available to Bank Millennium clients who have an account with the Bank and access to Millenet - Bank Millennium online banking system.


  • 2. How to file Family 500+ application at Bank Millennium?

    1. Before you start filling out the application

      • Prepare your personal data and personal data of the members of your household, including children (inter alia, name, surname, PESEL, marital status, name of the tax office for each person). Some of the data will be downloaded from the bank’s system.
      • prepare selected documents which confirm family’s income, disability or formal and legal situation of the family in the electronic version. The scope of the documents attached to the application depends on the individual situation of the applicant.

    2. Log to Bank Millennium online banking system.
    3. You can find the application on the main website, in the top left-hand corner. The application is also available in Applications and Agreements.
    4. Fill in the data in the application following the instructions on the screen.
    5. If required, enclose attachments to the application.
    6. Approve and close the application.
    7. Download your application confirmation certificate and copy of the application, and save them in a secure location. It is the only moment, when you can download a copy of your application filed through the bank.
    8. After the bank transfers your application to the competent governing authority in your municipality, you will receive Official Confirmation of Application Receipt/Submission (UPO/UPP) to the e-mail address given in the application. UPO/UPP e-mail message entitled: Urzędowe poświadczenie odbioru wniosku o świadczenie 500+ will come from the following e-mail address: upo500plus@mrpips.gov.pl.

    As soon as you receive UPO at the e-mail address, which you provided in your Family 500+ application, all questions and services related to your Family 500+ application will be handled by the governing authority in your municipality (for instance, City Social Welfare Centres).   • 3. Where to file Family 500+ application?

    You may file the application over the Internet:

    • in Bank Millennium online banking system
    • at ePUAP platform
    • at Platforma Usług Elektronicznych (PUE – online services platform) of ZUS
    • at emp@tia service
    • at online banking services of selected banks in Poland

    You may also file your Family 500+ application by mail or in person at the offices designated in municipalities (for instance, City Social Welfare Centres). Family 500+ application cannot be filed at the bank branch or bank info line.


  • 4. Do I have to be Bank Millennium client to file Family 500+ application through Bank Millennium?

    Yes, the application is available at Bank Millennium only for the persons who are the bank’s clients, have access to Millenet (online banking system) and have an account at Bank Millennium.


  • 5. I am Bank Millennium client and have access to online banking services. Why cannot I file my Family 500+ application?

    If you opened an account with Bank Millennium confirming your data with an authorisation transfer from another bank and you did not confirm your personal data at a branch, then you cannot submit the application for child care benefit through the Bank Millennium online banking system. If you confirm your identify at any Bank Millennium branch, you will be able to submit your Family 500+ application through Bank Millennium online banking system.

    Filing Family 500+ application will not be possible either when you do not have an account at Bank Millennium.

  • 6. How can I know to what governing authority in my municipality I should address the application?

    Request such information at your local municipality. At the end of the document you will find a list of info line numbers operating in individual voivodeships.


  • 7. What account should I state in my Family 500+ application for benefit pay-out?

    You may give any bank account on which you want to get benefits for your children. If you submit the application in Bank Millennium online banking system, the most convenient choice is your account at Bank Millennium. Apart from transfer to your account, you may also collect the funds in other ways – for information on what form of payout will be available at your municipality, if you do not have an account, contact the governing authority in your municipality.


  • 8. May I give child’s account for benefit payout in my Family 500+ application?

    The applicant may indicate any account to which the funds will be transferred. However, he or she indicates one account number for all the children.

    Importantly, a child up to 13 years of age cannot perform legal transactions connected to holding an account. Hence, even if such a child holds PKO Konto Dziecka for children up to 13 years of age and the parent wants the benefits from Family 500+ programme to be transferred to this account, then he or she manages the child’s account.


  • 9. Will it be possible to modify the application once it has been sent via the online banking service?

    No, if you want to modify/supplement the application, visit the governing authority in your municipality, which you indicated in the application.


  • 10. Should the application be filed by one or both parents?

    The application is filed by one of the parents. Marital status of the parents is not important, the support will be received by all the families — such where the parents live in a marital status or an informal union and these where one of the parents brings up children alone.

    If parents are divorced — the application is filed by the parent with whom the child lives. If the child lives with both parents, the application is filed by the parent who actually takes care of the child. If parents share the care of their child in accordance with the child care plan (on the grounds of the court decision establishing intermittent care) — then both parents may file the application.   • 11. What should I do if I cannot enclose attachments to my Family 500+ application?

    To your application for child care benefit within the Family 500+ programme, you may enclose attachments of specific parameters:

    • Attachment formats: JPG, PNG or PDF
    • Maximum number of attachments: 7 pieces
    • Maximum size of a single attachment: 500 KB
    • Maximum total size of all the attachments: 3,5 MB
    • Names of the attachments cannot contain Polish characters, special characters or spaces and they have to be different in a given application.

    In case of other problems in adding attachments to the application than those enumerated above, you may send you application without attachments and submit them later to the governing authority, upon the request of the head of the authority.


  • 12. How will I know that I have received UPO/UPP from a credible source?

    The e-mail message containing UPO/UPP entitled „Urzędowe poświadczenie odbioru wniosku o świadczenie 500+” will be sent by: upo500plus@mrpips.gov.pl.


  • 13. Who decides about granting the benefit to me? What is the role of the bank in this process?

    Decision about granting the benefit is taken by the governing authority in your municipality following an analysis of your application.

    The role of the bank is limited to verification of the applicant’s identity and enabling the Clients to file Family 500+ application in Millenet - Bank Millennium online banking system. The bank does not perform formal or technical verification of the data provided in the application and attachments, nor does it have any information on further processing of the application by the governing authority. The bank permanently deletes the application and attachments upon transferring the application to the national IT system - emp@tia.


  • 14. Are my data held by the bank taken into account when deciding about Family 500+ benefit?

    The bank helps you to fill out the Family 500+ application substituting your personal data which it holds, such as name, surname, PESEL and number of the account, provided you indicate it as the account to which the benefits from Family 500+ programme will be transferred. In the Family 500+ application the bank does not provide any information about the products and services held by the applicant at the bank.


  • 15. Does the bank transfer to the granting authority any data from my account?

    The bank helps you to fill out the Family 500+ application substituting your personal data the bank knows, such as name, surname, PESEL, number of the account at the bank. The bank submits them on a form to the benefit granting authority upon your approval to send the application. The bank sends also the number of your account, provided you indicate it as the account to which the benefits from the Family 500+ programme will be transferred.


  • 16. 16. On what grounds does the bank support sending applications for 500+ benefits?

    Bank Millennium signed an agreement with the Ministry of Family, Labour and Social Policy on filing applications in the Family 500+ programme via Bank Millennium online banking system.


  • 17. When can I expect payout of the benefit?

    You may submit the first application at any time between 01.04.2016 and 30.09.2017. If you do it, then:

    • until 01.07. 2016 inclusive – if the application is complete and filled out correctly — you will get a decision from the municipality on your application and a balancing payment for the past months since 1 April in case of positive decision. For instance, you submit your application on 28.06.2016. By 28.09.2016, at the latest, you will get balancing payment for the period since 01.04.2016, i.e. for April, May, June, July, August and September.

      Note: in case of the applications filed in the period from 01.04.2016 to 01.07.2016, the governing authority in the municipality has 3 months for review of each application and for payout of benefit in case of positive decision, counting from the day of filing.

    • after 01.07.2016 — in case of positive decision, you will receive your child support without the balancing payment for the past months. For instance, if you file your application on 15.07.2016, you will not receive the balancing payment and your payouts will start as of July.

      Note: in case of the applications filed after 01.07.2016, if the application is submitted by the 10th day of the month, you will receive the decision and your benefit will be paid out for this month no later than by the end of the month in question. If the application is filed after the 10th day of the month, you will receive the decision and the first payout of your benefit, with the balancing amount for the period starting from the month when your application was filed, by the end of the next month.

    Importantly, if your application is filled out incorrectly, is incomplete or its processing requires additional clarifications, the applicant will receive a written call from the governing authority in his/her municipality to correct the application, supplement it or submit clarifications.

  • 19. What is the difference between the confirmation of application receipt which I can download from the online banking service and Official Confirmation of Application Receipt/Submission - Urzędowe Potwierdzenie Odbioru/Przedłożenia (UPO/UPP)?

    Confirmation of the application receipt is issued by the bank in the Bank Millennium online banking service after filling out Family 500+ application. It confirms that the Bank has sent the application to emp@tia system, which then transfers it to the governing authority in the relevant municipality. Confirmation of application receipt may have a number of statuses:

    • in processing – which means that the bank is in the process of transferring the application to emp@tia system.
    • delivered - which means that the application was transferred to emp@tia system and further processing of the application rests with the governing authority in the municipality, which was indicated in the application by the applicant.
    • error in delivery – which means that the application was not transferred to emp@tia system due to data error, technical error etc. The data included in the application should be re-verified  and the application should be re-submitted. If the problem repeats, the application should be delivered to the governing authority by mail or in person.

    These statuses may change once a day and the final status should be displayed in the system after two days from sending the application.

    You will receive Urzędowe Potwierdzenie Odbioru/Przedłożenia (UPO/UPP) on the e-mail address given in the application when the application is registered in emp@tia system. Receipt of the confirmation from the bank and receipt of UPO/UPP do not warrant that you will receive child benefit. Each Family 500+ application is analysed by the governing authority in the municipality on a case-by-case basis for fulfilment of the statutory conditions for granting care benefit, while the governing authority in the municipality may, communicating with the applicant in writing, require delivery of additional clarifications or documents.

    NOTE! Granting or refusal to grant care benefit on the grounds of the application filed, are provided through administrative decision issued by the governing authority in your municipality. Applicant can appeal against this decision within the deadlines and under the procedure stipulated in the instructions for the received decision.


  • 20. What data does the technical confirmation of receipt of Family 500+ application contain?

    1. APPLICATION STATUS - status downloaded from emp@tia system
    2. APPLICATION IDENTIFIER – contains a unique application number
    3. DATE OF APPLICATION SUBMISSION – date of entering application in the sender’s system
    4. NAME – applicant’s data
    5. SURNAME - applicant’s data
    6. PESEL - applicant’s data
    7. NAME OF THE APPLICATION SENDER – contains information about the institution: bank
    8. NAME/CODE OF THE GOVERNING AUTHORITY – name/code of the institution - , recipient of the application – whose domain system is integrated with emp@tia system

  • 21. When and where from can I download the confirmation of receipt of Family 500+ application issued by the bank?

    You may download the confirmation of submission of your „Family 500+” application immediately after you submit the application in the online banking service. Confirmation status may change and it will be updated in the online banking system under „Umowy i Wnioski”.

    Once the application is transferred from the online banking system to emp@tia system, all questions and services related to your Family 500+ child care benefit application are taken over by the governing authority in your municipality competent for the applicant.


  • 22. May I, and how, collect a copy of my Family 500+ application filled out over the online banking system?

    You may collect a copy of the Family 500+ application only directly when you complete filling out your Family 500+ application in the online banking service – pursuant to the Act, the data contained in the application are removed from the system immediately upon their transfer.


  • 23. What statuses may Family 500+ application filed in the online banking have on the application confirmation?

    On the confirmation your Family 500+ application filed in the online banking system may have the following statuses:

    • in processing – which means that the bank is in the process of transferring the application to emp@tia system.
    • delivered - which means that the application was transferred to emp@tia system and further processing of the application rests with the governing authority in the municipality, which was indicated in the application by the applicant.
    • error in delivery – which means that the data included in the application should be re-verified  and the application should be re-submitted. If the problem repeats, the application should be delivered to the governing authority by mail or in person.

  • 24. Why the bank does not want to inform me about the status of my Family 500+ application?

    Bank Millennium supports only filing of Family 500+ application and its sending from the bank’s system to the governing authority in the municipality through emp@tia system operated by the Ministry of Family, Labour and Social Policy.

    As, upon transferring Family 500+ application to emp@tia system, the bank cannot store the data from the application, it is not able to inform about the status of the application after it has been sent to the governing authority in the municipality.   • 25. Does the bank accept claims concerning applications for „Family 500+” benefit?

    Bank Millennium is responsible for providing its clients with a functionality to access Family 500+ application via its online banking service and for correct transfer of the application to emp@tia system, which then submits it to the governing authority in a given municipality.

    As evidence of submission of your application to emp@tia system, the bank issues a technical confirmation of application submission. Hence, the bank may be liable for claims related to the transfer of your application to emp@tia system.

    As soon as you receive Official Confirmation of Application Receipt/Submission or Urzędowe Poświadczenie Odbioru wniosku/Przedłożenia (UPO/UPP) at your e-mail address, which you entered in your Family 500+ application, all your questions will be handled by the dedicated governing authority in your municipality, which will process your Family 500+ application (for instance, City Social Welfare Centres). In case of claims concerning Family 500+ application, please contact directly the governing authority competent for your residence.


  • 26. How will I find out that I filled out my Family 500+ application incorrectly?

    The bank is responsible for confirmation of your identity in the Family 500+ application, but it does not perform technical verification of the data and attachments which you entered or enclosed to your application.

    Confirmation of application submission which is displayed in the bank’s online banking service and UPO, which you receive at your e-mail address, confirms your formal submission of Family 500+ application, but not that it has been filled out correctly. If you have filled out 500+ application incorrectly, you will receive by an e-mail request from the governing authority in your municipality to adjust the application/supplement the information/documents.


  • 27. Another child has been born in my family. How to file the application for Family 500+ child care benefit through Bank Millennium?

    You cannot adjust/update Family 500+ application that has been already filed through the bank. If the situation in your family has been changed, contact immediately the governing authority in your municipality.

    However, if you want to apply for the benefit for a child whom you so far have not included in the application, for instance, for a newly born child, then you are welcome to file another application for this child through the bank.


  • 28. My child stopped to qualify for the benefits from the Family 500+ programme. How to file an adjustment / update to the Family 500+ application through Bank Millennium?

    You cannot adjust/update Family 500+ application through the bank. If the situation in your family has been changed, contact immediately the governing authority in your municipality.


  • 29. What bodies are involved in the services related to your Family 500+ application filed through Bank Millennium? What is the role of the bank, emp@tia system and the governing authority in the municipality?

    • Bank Millennium verifies applicant’s identity and enables its clients to submit Family 500+ application in the online banking system. You cannot file the application at a branch or through info line consultant. The Bank supports download of a filled out Family 500+ application ONLY directly upon filing the application. However, technically, confirmation of application receipt can be downloaded for a period of 10 years from the moment of sending the application. The Bank does not perform formal or technical verification of the data stated in the application and attachments and does not have information about further service of the application supported by selected governing authority. The bank deletes the application and attachments upon transferring the application to emp@tia system.

    • Emp@tia system is an IT system of the Ministry of Family, Labour and Social Policy for the social welfare services which is responsible for transferring Family 500+ application from the bank to selected governing authority in the municipality for further processing. Emp@tia system sends to the applicant UPO to the e-mail address stated in the application.

    • Governing authority in the municipality / competent authority is the body selected for processing child care benefit applications within the Family 500+ programme (for instance, City Social Welfare Centres). You select the governing authority in your municipality in accordance with your living address.


  • 30. How to change the number of the account, to which money from Rodzina 500+ programme will be sent?

    If you want to change the account, to which money from Rodzina 500+ programme is to be sent, contact the governing body in your municipality, which you indicated in the Rodzina 500+ application.


  • 31. Whom can I contact about Family 500+ programme?

    First of all, contact the governing authority at the municipality where you live – it is the body that will review your application, issue administrative decision in your case and will pay the benefit in case of positive decision.

    The info line and located in each voivodeship and they will support the Programme for three months.
