In person at Millennium DM Customer Service Point (CSP)

STEP 1: please read the following:

  • rules and regulations governing our brokerage services,
  • agreement for execution of buy/sell orders of financial instruments and holding cash and securities accounts,
  • information required by MiFID,
  • fees and commissions.

STEP 2: please prepare the following documents and information:


for residents:

  • personal ID form or passport,
  • NIP (tax identification) number,
  • bank account numbers for transfers from the investment account to other bank accounts in Poland - only if Investor wishes to use remote transfer option,

for non-residents:

  • passport,
  • bank account numbers, as above.

STEP 3: please go to one of our 12 Millennium DM CSPs
Additional information - what you should know about the documents and rules for executing agreements at CSP.

  • for agreement for spouses with community property regime - the above requirements apply to both spouses,
  • documents to complete:
    • agreement for execution of buy/sell orders of financial instruments and holding cash and securities accounts,
    • Investor Information Form - compliant with market standard (required by MiFID),
    • Investor Signature Specimen Sheet

      and optionally:
    • form with password - only if Investor activates phone order option,
    • application for internet service conditions - only where Investor activates internet service access for a proxy,
    • appendix with bank accounts - only if Investor activates phone or internet transfer orders from investment account to bank accounts,
    • power of attorney to manage the account - only if given to third party,
    • financial situation statement - only if agreement for derivatives or DPD annex is signed simultaneously,
    • spouse consent - only if agreement for derivatives with maximum concentration limit (MLZ) larger or equal to PLN 300,000 or DPD annex is signed, and no spousal versions are signed,
  • if phone order option is activated - the service is activated at CSP once the agreement is signed,
  • if internet orders are activated:
    • the service is activated on the day following the agreement signature; at Investor's request, the date will be changed to the agreement signature date,
    • login and password of Investor's choice may be collected from CSP or sent by mail,
  • if the agreement is signed by a proxy, please prepare a power of attorney to execute the agreement. Investor's signature on the power of attorney should be authenticated by a notary public or a Polish diplomatic post, or bear apostille of foreign country official - as prescribed by the Hague Convention of 5 October 1961 abolishing foreign attestation requirement of official documents; such power of attorney should contain:
    • authorisation for proxy to grant Millennium DM a power of attorney for Millennium DM contained in the agreement for execution of buy/sell orders of financial instruments and holding cash and securities accounts (see STEP 1 above),
    • authorisation for communication of information necessary to complete Investor Information Form to Millennium DM, with the proviso that information necessary to complete Investor Information Form (as Investor's knowledge statement) must be provided in such power of attorney,
    • if the power of attorney does not contain the authorisation referred to in the previous section, authorisation to forward to Millennium DM the above form completed by Investor, with the proviso that Investor's signature on the form should be notarised or put in the presence of Millennium DM employee,
    • if under the agreement Investor intends to place phone and/or fax and/or internet instructions, also a power of attorney for: 
      - filing an application for phone and/or fax password, 
      - filing an application for the internet service authorisations,
      - receiving logins and passwords (also token, if any),
  • proxy may give instructions after filing signature specimen with Millennium DM,
  • Millennium DM charges no fee for opening an account; account maintenance fee is:
    • PLN 0 - if in a given quarter there were no operations in the account effected according to Investor's instructions,
    • PLN 13 quarterly - in other instances.
